Expand your search with Express!Our client in Ridgely is seeking a Housing Services Case Manager!Responsibilities, including, but are not limited to:
Conduct intake, assessment, and orientation to determine program eligibility in accordance with grant and funding guidelines.
Conduct weekly case management meetings with all shelter guests, hotel clients, and Rapid Rehousing Program participants.
In partnership with participants, develop Individualized Housing Stability Plans (IHSP) and monitor progress.
Coordinate weekly case conferences to receive updates on each guest and determine their needs.
Work with clients to coordinate transportation needs (schedule taxis, staff and volunteer drivers).
Identify and recruit landlords and/or property management companies for housing opportunities.
May perform unit inspections and home visits.
Utilize a trauma-informed, strengths-based approach in working with participants to reduce barriers, achieve short- and long-term goals, and secure and maintain permanent housing as participants strive toward personal and financial self-sufficiency.
Maintain accurate data, files, case notes and other materials electronically (HMIS) and hard copies, in accordance with established methods and procedures and grant requirements.
Conduct financial literacy and life skills empowerment classes, groups, and individual sessions utilizing empowerment materials.
Complete emergency telephone intakes and provide case management, resources, and referrals, as necessary.
Attend regularly scheduled agency, team, and community meetings.
Coordinate with a variety of outside service providers to connect participants with available community resources.
Conduct all functions in accordance with funding agency guidelines, policies and proceduresRotate on-call responsibilities with Martin's House team members.
Other duties as assigned.
Knowledge, Skills & AbilitiesKnowledge and understanding of the needs and barriers associated with homeless and formerly homeless individuals.
Acceptance and understanding of individuals from diverse backgrounds with urgent and multiple barriers to self-sufficiency.
Ability to quickly develop a rapport, inspire, motivate, and coach participants towards achieving their goals, and maintains professional boundaries.

Works well in a team and independently.
Excellent verbal and written communication skills.
Organized, detail-oriented, and can coordinate multiple demands in a outcomes-based environment.
Ability to write standardized clear, and concise case notes.
Proficient in Microsoft Word, Excel, PowerPoint, Publisher, and Access.
QUALIFICATIONSBachelor's degree in Social Work, Psychology or similar discipline highly desirable.
Experience may be substituted for college for up to two years, on a year for year basis.
At least two years experience in case management for individuals and families experiencing or have previously experienced homelessness.
Valid driver license and auto insuranceMust be able to pass criminal background screening and credit check.
